CASE OF A 48 YEARS OLD FEMALE WITH DKA AND CVA

 This is an E logbook to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here, we discuss our individual patient's problems through series of inputs from an available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence-based inputs. CASE DISCUSSION :

This is the case of a 48 years old female who came to the hospital on 10th May, 2021 with :

CHIEF COMPLAINTS :

1) LEFT SIDED WEAKNESS (BOTH UL & LL) SINCE 3 DAYS

2) ALTERED SENSORIUM SINCE 10th  MORNING

3) ABDOMINAL DISTENSION SINCE 10th MORNING

HISTORY OF PRESENT ILLNESS :

Patient was apparently asymptomatic 3 days back then she developed left sided weakness which was gradual and associated with tingling sensation so she was admitted in the outside hospital and there the treating doctor found acute infarcts on CT scans and I/V/O uncontrolled sugars patient was referred to the present hospital for further evaluation.

HISTORY OF PAST ILLNESS :

No H/O similar complaints in the past

K/C/O DM Type-2 since 2 years

K/C/O HTN since 3 days (No anti-hypertensive medication was taken)

PERSONAL HISTORY :

Appetite : Normal

Diet: Mixed

Bowel habits : Regular

Micturition : Abnormal

No known allergies

No addictions

FAMILY HISTORY : Insignificant


GENERAL EXAMINATION :

Patient was examined in a well-lit room after obtaining a valid consent 

Pallor: Absent

Icterus: Absent

Cyanosis: Absent

Clubbing: Absent

Lymphadenopathy: Absent

Oedema: Absent


VITALS:

Temperature: Afebrile

Pulse Rate: 127 bpm

Respiratory Rate: 40 cpm 

Blood Pressure: 150/90 mmHg

SPO2 : 92% at RA

GRBS: 453 mg%

  DIABETIC KETOACIDOSIS WITH CVA WITH ACUTE INFARCT IN THE RIGHT PARAMEDIAN-PONS WITH TINY CALCIFIED GRANULOMAS IN LEFT FRONTAL AND TEMPORAL BONES

   INTUBATION NOTES :

   In view of falling saturations, patient was intubated with E.T tube 6.5 and kept on mechanical ventilation ( ACMV mode) on 11th May.

 On 13th May, patient had a cardiac arrest and CPR was initiated and continued for 8 cycles.
    CPR NOTES: 8 CYCLE OF CPR DONE BETWEEN 12:30 PM-1:00PM
        AT EACH TIME ,
  •  BP and PR : NON RECORDABLE
  • CPR INITIATED
  • INJ.ADRENALINE 1CC 
  • INJ.ATROPINE 1 CC GIVEN
     Inspite of all the above efforts , patient could not be revived and declared dead on 13/5/21 at 1:02pm as ECG showed no electrical activity.



Immediate cause of death : Acute Respiratory failure with AKI secondary to ? sepsis
Antecedent cause of death: Diabetic Ketoacidosis with acute infarct in right paramedian pons and tiny granulomas in left frontal and temporal lobes with ? viral pneumonia
